My guess is a tragic accident. Carriage over-turned? Baby dropped? I know it resembles a hoof mark but a hoof would have done much much more damage than this.

Little girl, Julia Taylor, with her doll. So heartbreaking... So many children died young back then, before penicillin and whatnot; a cemetary from that era is startling—just about half, or a third are graves of young children, ugh.
