Komorbid med nevrodegenerative lidelser som Parkinson, Lewy-legeme demens, Downs syndrom,  multippel systematrofi, narkolepsi, multippel sklerose. RBD som første manifestasjon/predikativ av begynnende nevrodegenerativ lidelse. Symptomer kan utløses av medikamentell REM-suppresjon: SSRI, TCA, MAOi.
